The organisms usually involved are opportunists already present in most systems, becoming invasive when the fish's defences are compromised by poor water quality, crowding, injury or stress. Because several genera produce the same appearance and susceptibility patterns differ between them and between isolates, treating on appearance alone is a matter of chance — and the widespread availability of over-the-counter antibacterials in some markets has made resistance a real and growing problem. Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

How to tell it apart: Test the water. Ammonia and nitrite exposure produce reddened gills and fins and rapid deterioration across the whole tank, and are both far commoner and instantly checkable. They are also frequently the reason a bacterial infection took hold, so a positive test does not exclude infection — it identifies the cause of it.
How to tell it apart: Many fish have naturally red fins or visible vessels, particularly in unpigmented and albino strains where blood is simply more visible. Compare with the same fish a month ago and with its tankmates; septicaemic streaking appears and progresses.
How to tell it apart: A single lesion, consistent with an identifiable cause, that heals rather than spreading and is not accompanied by systemic signs.
How to tell it apart: Mycobacteriosis is chronic, with progressive wasting and sometimes spinal curvature over months, rather than an acute event over days. It also carries a documented human health risk that ordinary bacterial septicaemia does not, and it requires laboratory confirmation.
How to tell it apart: Columnaris produces pale surface lesions and fin margin erosion rather than deep ulcers and vascular streaking, though the two can occur together.

Which agent is appropriate depends on the organism and its susceptibility, which is why culture and sensitivity testing rather than guesswork is the veterinary standard. In most jurisdictions the effective agents are prescription-only, and that restriction exists for good reason.
What it can damage: Antibacterial products suppress nitrifying bacteria and commonly cause an ammonia spike, which worsens the water quality that caused the problem. Several affect invertebrates. Inappropriate use selects for resistant organisms, which is a documented and worsening problem in ornamental aquaculture.
This is the presentation where veterinary involvement changes the outcome most, because identifying the organism and its susceptibility is the difference between treatment and guesswork.
Where a fish has extensive ulceration, is not feeding and is deteriorating, humane euthanasia discussed with a veterinarian is a legitimate and sometimes the kindest outcome.
