The name implies a single condition with a single treatment, and that framing is responsible for a great deal of unnecessary medication. Eroding fins are the visible end point of several processes: sustained exposure to ammonia, nitrite or high organic load; repeated physical damage from a fin-nipping tankmate or sharp decor; a secondary bacterial colonisation of tissue already damaged by one of those; or, occasionally, a primary bacterial infection such as columnaris. The distinguishing question is not what the fins look like but how fast it is progressing and what else is in the tank.
Short answer
Fin rot is an appearance, not a disease: fins that are eroding, fraying or losing tissue from the edge inwards. It has several quite different causes, and the two commonest — poor water quality and damage from tankmates — are not infections at all. Treating it with an antibacterial without establishing which cause applies is the usual mistake.

Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

How to tell it apart: Test ammonia, nitrite and nitrate. Erosion driven by water quality typically affects several fish, progresses slowly, has no localised inflamed focus, and begins to improve within days of a serious water-change regime with nothing else done. This is the first thing to exclude and the commonest answer.
How to tell it apart: Damage is asymmetric, appears as clean bites or splits rather than uniform recession, affects the fish with the longest fins while others are untouched, and stops when the culprit is removed. Watch the tank after lights-on and at feeding time — nipping is rarely observed unless it is looked for.
How to tell it apart: A single tear in one fin, with clean edges, that heals rather than progressing.
How to tell it apart: Regrowing fin tissue is thin, clear and often slightly irregular, and it extends the fin rather than shortening it week on week. Photograph the fish and compare after seven days — direction of travel is the whole answer.
How to tell it apart: Columnaris progresses over hours to days, not weeks, and typically shows a pale greyish margin or a body lesion as well. Rapid progression is the signal to stop assuming this is ordinary fin rot.
How to tell it apart: Suggested by reddening and inflammation at the margin, and by continued progression after water quality has been corrected and physical causes removed. This is the case in which antibacterial treatment is genuinely indicated.
