The lesions are distinctive — small pits on the head that enlarge, sometimes extending along the lateral line. What causes them is not. Spironucleus is commonly recovered from affected fish, and antiprotozoal treatment often helps, which supports a parasitic contribution. But the organism is also found in fish with no lesions, lesions occur in fish with no detectable organism, and dietary deficiency and chronically poor water quality are consistently associated. The activated-carbon hypothesis is widely repeated in the hobby and not, as far as AquariumHQ can establish, supported by controlled work. How to tell it apart: Injury produces asymmetric damage in a location consistent with a collision or a fight, and it heals rather than progressing. The syndrome produces multiple symmetrical pits that enlarge over weeks.
How to tell it apart: Cichlids have normal sensory pores on the head in a regular pattern. Normal pores are symmetrical, uniform, present since the fish was young, and do not enlarge. This is the commonest false alarm, and the useful test is whether they are changing.
How to tell it apart: Bacterial ulcers are typically reddened and inflamed at the margins and progress over days. The syndrome's pits are relatively bloodless and progress over weeks.
How to tell it apart: Mycobacteriosis produces wasting, spinal deformity and internal lesions alongside skin changes, and requires laboratory confirmation. Where a fish has both head pitting and progressive wasting, this needs excluding.
