Swim bladder disorder

The swim bladder is a gas-filled organ used to hold neutral buoyancy. A fish that cannot control it may float helplessly at the surface, sink and struggle to rise, or roll onto its side or back. The causes divide usefully into three groups. Mechanical: a distended gut, constipation or gulped air pressing on the organ, which is common and often correctable. Structural: the compressed body shape of selectively bred round-bodied goldfish and similar strains, which is permanent and a consequence of how the fish was bred. And pathological: infection, injury or damage to the organ or its nerve supply, which needs veterinary assessment. Which group applies determines everything, and the appearance alone does not distinguish them.

Short answer

Swim bladder disorder is a description of a fish that cannot control its buoyancy — floating, sinking or rolling. It is not one disease. The causes range from a full gut pressing on the organ, through selective breeding that has deformed it, to infection, injury and neurological damage, and they have completely different responses.

This is a description of how a fish looks, not a diagnosis. Several unrelated causes produce it, and treating the appearance without establishing the cause is the commonest way keepers make the outcome worse.

Signs

  • Floating at the surface and unable to swim down
  • Sinking to the bottom and unable to rise without continuous effort
  • Rolling onto one side or swimming upside down
  • Head-down or tail-up swimming posture
  • Struggling in an otherwise alert, feeding fish — buoyancy problems often leave the fish's appetite intact, which is diagnostically useful
  • In round-bodied ornamental goldfish varieties, a long-standing tendency rather than a sudden change

Do this first: the environmental response

Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

  1. Test ammonia and nitrite first, because poisoning mimics this and is dangerous to the whole tank.
  2. Check the temperature is appropriate for the species. Goldfish kept cold digest slowly, and this is a common contributor.
  3. Reduce water depth or provide resting places near the surface if the fish cannot swim down, and ensure any fish floating at the surface is not left with exposed skin drying out.
  4. Reduce flow, which an unstable fish cannot fight.

Non-disease explanations — check these first

  • How to tell it apart: The fish is otherwise alert and feeding, the abdomen looks full, the problem fluctuates through the day and is worse after feeding. This group frequently improves within days of withholding food briefly and then changing the diet. It is the commonest cause and the most correctable.

  • Body shape of a selectively bred variety

    How to tell it apart: A round-bodied fancy goldfish that has always struggled somewhat, with no acute change, is showing the consequence of its body shape. This is not curable, and management — sinking foods, appropriate water depth, no strong flow — is the honest answer rather than treatment.

What makes it more likely

Each entry states the mechanism, not just the association — an association without a mechanism is not something you can act on.

  • Round-bodied selectively bred varieties. Selection for a shortened, globe-shaped body compresses and distorts the internal organs including the swim bladder. In these strains buoyancy problems are a predictable consequence of the body shape rather than an illness, and they are frequently not correctable.
  • Overfeeding, particularly of dried food. A distended gut presses on the swim bladder. Dried foods that expand after being eaten, and food taken at the surface with gulped air, both contribute.
  • Constipation. The same mechanical mechanism, sustained over longer periods.
  • Cold water for the species. Slows gut transit, which makes mechanical causes more likely, and is a frequent contributor in unheated goldfish tanks.
  • Infection or physical injury involving the organ or its nerve supply. Direct damage or inflammation. This group needs veterinary assessment and is not addressed by feeding changes.

Treatment categories

AquariumHQ names classes of response and never doses. Product concentrations differ between markets and formulations, several actives are temperature- and pH-dependent, and the invertebrate and plant tolerances that decide whether the rest of the tank survives are not printed on the label. Where a category puts a chemical in the water, what it can damage is stated with it.

Dietary management

For the large mechanical group, a short fast followed by a change of diet — sinking rather than floating food, pre-soaked dried food, and more plant material or appropriate live and frozen food for species that take it — resolves or substantially improves the problem. AquariumHQ gives no fasting schedule: the appropriate period differs by species, size and temperature, and prolonged withholding of food is itself harmful.

Correct the husbandry

Correcting temperature, flow, water depth and feeding method addresses the conditions in which mechanical buoyancy problems arise and recur.

Supportive care

For structural cases in selectively bred varieties, management rather than cure: sinking food, reduced depth, low flow, and protection from surface drying. Many such fish live well for years with this.

Veterinary referral

Appropriate where the onset was sudden in a previously normal fish, where systemic signs accompany it, or where feeding changes have not helped — those are the cases where the cause is not mechanical.

Isolation and quarantine

  • A fish floating at the surface is at risk of skin damage from air exposure, and one stuck on the bottom may be harassed. Lowering the water level or providing a quiet space can help.
  • Isolation is about protecting the affected fish, not about protecting the others — this is not transmissible.

When a vet is the right call

  • Sudden onset in a previously normal fish
  • Accompanying loss of appetite, reddening or lethargy
  • No improvement after dietary and husbandry correction
  • A fish unable to feed or rest normally, where welfare needs assessing

  • Feed sinking or pre-soaked food to species prone to gulping air at the surface, particularly fancy goldfish.
  • Avoid overfeeding, which is the commonest contributor.
  • Keep the temperature appropriate for the species — cold goldfish digest slowly.
  • Think carefully before buying heavily modified round-bodied varieties: buoyancy difficulty is a predictable consequence of the body shape, not bad luck.
