Dropsy

The scales protrude because fluid has accumulated in the tissues and body cavity faster than the fish can excrete it. Osmoregulation is a continuous, active process in a freshwater fish: it lives in water far more dilute than its own body fluids and must constantly pump out the water that enters by osmosis, work done largely by the kidney. When that fails, water accumulates. The failure can follow bacterial infection, kidney damage from any cause, mycobacteriosis, a tumour compressing organs, or heart failure. Prognosis is poor by the time pineconing is visible, and AquariumHQ says so rather than offering a treatment for an appearance.

Dropsy is not a disease. It is a description of a fish whose body has swollen with retained fluid until the scales stand out from the body like a pine cone. That appearance is the end stage of several unrelated processes — kidney failure, systemic infection, organ damage, a tumour — and the pineconing tells you that fluid regulation has failed, not why.

Signs

  • Abdominal swelling, sometimes markedly asymmetric
  • Scales standing out from the body, most obvious when the fish is viewed from directly above — this is the defining observation and it is easy to miss from the side
  • Protruding eyes in some cases
  • Reduced appetite and lethargy
  • Pale or stringy faeces in some cases
  • Gill pallor in some cases
  • The fish sitting on the bottom or struggling to maintain position

Do this first: the environmental response

Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

  1. Test ammonia, nitrite and nitrate. Chronic exposure is a documented route to the kidney damage that produces this, and it also tells you whether the other fish are at risk.
  2. Improve water quality substantially and sustain it — this is supportive rather than curative, but poor water actively worsens an osmoregulatory problem.
  3. Keep the temperature stable and within the species' range.
  4. Reduce stress: dim lighting, minimal disturbance, and no netting unless necessary.

  • A gravid female

    How to tell it apart: A female carrying eggs is swollen but her scales lie flat. View from directly above: if the outline is smooth, this is not dropsy. This is much the commonest false alarm, and it leads to fish being medicated for being pregnant.

  • Overfeeding or a full gut

    How to tell it apart: A well-fed fish is rounded but smooth-scaled, and the swelling changes over a day. Dropsy does not resolve overnight and the scale profile is the discriminator.

Other conditions

  • How to tell it apart: Constipation produces abdominal distension with flat scales and often trailing faeces or none at all, and responds to a change in feeding. Scale protrusion is not part of it.

    How to tell it apart: Typically produces asymmetric, firm swelling that grows steadily in an otherwise well fish, often over months.

What makes it more likely

Each entry states the mechanism, not just the association — an association without a mechanism is not something you can act on.

  • Chronic poor water quality. Sustained exposure to ammonia, nitrite or high nitrate damages the kidney over time, and the kidney is the organ that does the osmoregulatory work.
  • Systemic bacterial infection. Infection can damage the kidney and other organs directly, and is one of the commoner processes ending in this appearance.
  • Mycobacteriosis. Chronic granulomatous disease of the internal organs can end in osmoregulatory failure, and is a differential worth taking seriously because of its human health dimension.
  • Age and tumours. Organ failure and neoplasia are ordinary causes of this appearance in an old fish, and neither is treatable.
  • Selectively bred body shapes. Round-bodied ornamental strains have compressed internal organs, which is associated with a higher incidence of organ failure and makes swelling harder to assess.

Treatment categories

AquariumHQ names classes of response and never doses. Product concentrations differ between markets and formulations, several actives are temperature- and pH-dependent, and the invertebrate and plant tolerances that decide whether the rest of the tank survives are not printed on the label. Where a category puts a chemical in the water, what it can damage is stated with it.

Correct the husbandry

Clean, stable water reduces the osmotic and metabolic burden on a fish whose regulation has failed. It is the one thing that is always appropriate.

Veterinary referral

This is a presentation, and only a veterinarian can establish which process produced it. That is also the only route to a treatment that has any prospect of working, since treating the appearance cannot.

Supportive care

Where the fish is still feeding and comfortable, supportive care and good conditions are reasonable while the situation is assessed.

Consider humane euthanasia

Prognosis once scales are visibly protruding is poor. Where a fish is no longer feeding, cannot maintain position, or is visibly distressed, humane euthanasia is the appropriate outcome and should be discussed with a veterinarian. AquariumHQ regards prolonging this state with successive untargeted treatments as a welfare problem in itself.

Isolation and quarantine

  • Isolating an affected fish is reasonable — it allows close observation, individual feeding and, if it comes to it, a humane end without an audience of tankmates.
  • Isolation is not about protecting the others: the appearance itself is not transmissible, though some of its underlying causes are.
  • Where several fish develop this appearance, the tank rather than the fish is the problem, and water quality is the first thing to examine.

  • Accompanying wasting or spinal curvature, raising mycobacteriosis
  • To discuss humane euthanasia where the fish is deteriorating

  • Avoid chronic overstocking and under-maintenance.
  • Feed a varied, appropriate diet and avoid persistent overfeeding.
  • Quarantine new fish, which reduces the introduction of the infections that can end this way.
  • Be cautious about heavily modified round-bodied ornamental strains, whose internal anatomy is compressed by selection.
