Reducing density and correcting water quality removes the conditions that allowed a tolerated burden to become a disease. In a proportion of cases this is sufficient on its own.
Monogeneans are host-attached flatworms with a hooked attachment organ. The gill-dwelling genera are mostly egg-laying, which matters practically: eggs are not affected by treatments that kill the adults, so a single treatment leaves a second generation to hatch. Low burdens are common and tolerated on healthy wild and aquarium fish; disease follows when crowding, poor water quality or stress lets the population build. That last point is the reason a fluke diagnosis is rarely the whole story — the burden usually rose because something else was wrong first.
Short answer
Gill flukes are small monogenean flatworms that attach to the gill filaments with hooks. They cannot be seen with the naked eye, and the signs they produce — laboured breathing, flashing, one gill cover held open — are shared with ammonia poisoning, nitrite poisoning, low oxygen and several other parasites. A confident identification requires a gill scrape under a microscope.
Needs a microscope to confirm
This cannot honestly be identified by eye. Confirmation needs a skin or gill scrape examined under a microscope, which a veterinarian or an experienced fish-health service can do. Treatments aimed at it are frequently given on a guess.
Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

How to tell it apart: Test the water. This is the most important differential on the page: gill irritation from ammonia or nitrite produces exactly the same breathing pattern and flashing, affects every fish in the tank at once, and is far more common than flukes. A test kit distinguishes them in five minutes and no microscope distinguishes them faster.
How to tell it apart: Affects all fish simultaneously, is worst at the warmest part of the day or after a filter stoppage, and improves within minutes of adding vigorous surface agitation. A parasite burden does not improve in minutes.
How to tell it apart: Velvet involves the gills too, but adds a fine gold dusting on the body visible under a torch. Look at the flanks in a darkened room before concluding the problem is confined to the gills.
How to tell it apart: Typically follows a period of poor water quality or high organic load, and the gill tissue appears swollen and clubbed rather than carrying visible parasites. Distinguishing this from flukes genuinely requires microscopy.
How to tell it apart: Skin-dwelling monogeneans produce flashing and body irritation with less respiratory involvement. The two groups differ in reproduction — the skin genera bear live young and the gill genera lay eggs — which changes how long treatment must continue. Only a microscope separates them.
Each entry states the mechanism, not just the association — an association without a mechanism is not something you can act on.

Antiparasitic products active against monogeneans are the standard veterinary response where the identification is confirmed. Because the gill genera lay eggs that treatment does not reach, the course has to be repeated to cover hatching.
What it can damage: Products active against flatworms are also active against other invertebrates: snails in particular are frequently killed, and some formulations affect shrimp. Dying parasites and any invertebrate losses add to the organic load in a tank whose fish are already short of oxygen.
A gill scrape examined under a microscope is the only way to establish this, and it also identifies which parasite is present, which decides what will work.
