Oxygen enters aquarium water almost entirely at the surface, so gas exchange depends on surface area and surface movement rather than on the volume beneath. Warmer water holds less oxygen while simultaneously raising the fish's metabolic demand, which is why hot weather is the classic setting. Other common causes are a stopped or restricted filter, a bacterial bloom consuming oxygen, an overstocked tank, a medication that reduces oxygen or increases demand, and — in a heavily planted tank — the hours before dawn, when plants have been respiring all night without photosynthesising.
Short answer
Oxygen deprivation occurs when dissolved oxygen falls below what the fish in the tank need. It is one of the few aquarium emergencies that can be substantially improved within minutes, by increasing surface agitation — which also makes it the most useful thing to try while you work out what is actually going on.

How to tell it apart: Test the water. Both produce identical gasping. The practical discriminator is that hypoxia improves within minutes of vigorous aeration and poisoning does not — so increasing aeration is both a treatment and a diagnostic test.
How to tell it apart: In a planted tank with CO₂ injection, fish gasping in the afternoon with a drop in pH through the day points to CO₂ rather than to low oxygen. Turn the injection off and increase surface movement.
How to tell it apart: Gouramis, bettas and other anabantoids breathe atmospheric air as a matter of normal biology and go to the surface routinely. This is not a symptom in those species. Look at the other fish in the tank — if they are behaving normally, nothing is wrong.
How to tell it apart: Fish that associate a person approaching with food gather at the surface and mouth at it. They disperse when you step back; gasping fish do not.
