Internal parasites

The label is used in the hobby as though it named a condition with a treatment, and it does neither. The groups it covers respond to entirely different compounds: a preparation effective against nematodes does nothing to a flagellate, and one effective against flagellates does nothing to a tapeworm. Confirming which is present requires faecal examination or necropsy. Meanwhile the sign that sends most keepers looking for internal parasites — white stringy faeces — has a large and much more common non-parasitic differential, which is that the fish has not been eating. A fish producing mucus-only faeces because it is off its food is producing exactly the appearance the hobby reads as worms.

Short answer

'Internal parasites' is not a diagnosis. It is a category covering nematodes, cestodes, trematodes, intestinal flagellates and microsporidians — organisms with different biology that respond to different treatments and share only the fact that they are inside the fish where nobody can see them.

Signs

  • Wasting despite continued feeding — the most useful single sign

  • Abdominal swelling in some infections, the opposite of wasting
  • Reduced activity and faded colour
  • Losses spread over weeks rather than clustered

Do this first: the environmental response

Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

  1. Establish whether the fish is eating. This is the single most informative observation and it costs nothing.
  2. Test water quality — poor conditions both mimic and worsen this picture.
  3. Increase water changes and remove detritus, reducing the concentration of infective stages.
  4. Feed well: affected fish are undernourished and need support regardless of what is causing it.

    How to tell it apart: Chronic wasting with spinal curvature over months, not responding to worming. It also carries a documented risk to the keeper, which nothing else in this category does, so it is the differential worth excluding first.

  • Hexamita and intestinal flagellates

    How to tell it apart: Frequently accompanied by head pitting in cichlids. Responds to a different class of compound from the worming preparations used against nematodes, which is why 'internal parasites' as a single treatment target fails.

What makes it more likely

Each entry states the mechanism, not just the association — an association without a mechanism is not something you can act on.

  • Wild-caught stock. Wild fish carry parasite burdens that a closed captive population would not, and the burden becomes clinically significant under transport and crowding stress.
  • Absence of quarantine. These infections are invisible for long periods, so an unquarantined fish can seed a tank months before anything is noticed.
  • Live food from uncontrolled sources. Several parasites use invertebrate intermediate hosts, and collected live food is a documented route.

Treatment categories

AquariumHQ names classes of response and never doses. Product concentrations differ between markets and formulations, several actives are temperature- and pH-dependent, and the invertebrate and plant tolerances that decide whether the rest of the tank survives are not printed on the label. Where a category puts a chemical in the water, what it can damage is stated with it.

Correct the husbandry

Correcting feeding, competition and water quality resolves a substantial proportion of cases that present this way, because a substantial proportion of them are not parasitic.

Anthelmintic (worming)

Where a nematode or cestode infection is established, worming preparations are the appropriate class. Which one depends on the organism, and a preparation effective against one group is frequently useless against another — which is why treating the label rather than the organism so often fails.

What it can damage: Worming actives are toxic to ornamental snails and several affect shrimp. In many jurisdictions the effective preparations are prescription-only. Repeated speculative courses select for resistance and stress fish that are already in poor condition.

  • Speculative treatment before establishing that the fish is eating normally
  • Sequential trials of different products, which is common practice and does more harm than the parasites
  • Tanks with valued snail populations

Veterinary referral

Faecal examination establishes which group is present, which is the difference between treatment and guesswork. This is the category where that difference is largest.

Isolation and quarantine

  • Isolating one visibly affected fish rarely protects the rest, because transmission has usually already occurred within the tank.
  • Remove dead fish promptly — scavenging is a documented transmission route for several of these organisms.
  • Isolation is useful for observing whether an individual is actually eating, which is the question that most often resolves the case.

When a vet is the right call

  • Wasting in a fish that is confirmed to be eating
  • Any decision to medicate, since the class of compound depends on the organism
  • Failure to respond to a first course, which usually means the group was misidentified
  • Accompanying spinal curvature, which raises mycobacteriosis and its human health dimension

AquariumHQ describes what is commonly reported and what husbandry correction is usually indicated. It does not diagnose and is not a substitute for an aquatic veterinarian. Many treatments are given to healthy fish suffering from a water-quality problem, which makes the outcome worse rather than better.

  • Quarantine every new fish, and quarantine wild-caught stock for longer.
  • Remove dead fish immediately.
  • Be cautious with live food from uncontrolled sources.
  • Watch body condition rather than waiting for a visible sign — wasting in a fish that is eating is the earliest reliable indicator.
