Popeye (exophthalmia)

The eye protrudes because fluid or gas has accumulated behind it, or because the tissues around it are inflamed. That can follow a physical injury, a localised infection, a systemic bacterial infection, chronic poor water quality, gas supersaturation, or in rare cases a tumour. The unilateral-versus-bilateral distinction is genuinely useful triage and costs nothing to make. Where the eye itself has become cloudy as well, corneal damage is involved and the prognosis for vision in that eye is poorer.

Short answer

Popeye describes an eye protruding from the socket. It is a sign, not a diagnosis, and the most useful first observation is whether one eye is affected or both: one eye usually means injury or a local infection, while both eyes more often point to a systemic problem or to the water.

This is a description of how a fish looks, not a diagnosis. Several unrelated causes produce it, and treating the appearance without establishing the cause is the commonest way keepers make the outcome worse.

Signs

Do this first: the environmental response

Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.

  1. Test ammonia, nitrite and nitrate. Bilateral cases in particular point at the water.
  2. Increase water changes and sustain the improvement — the eye recovers slowly and needs clean water throughout.
  3. Remove sharp decor and identify any aggressive tankmate.
  4. Where several fish are affected and bubbles are visible, check for a pump drawing air or a recent change in the water supply.

Compare with a photograph of a healthy example of the same variety before concluding anything is wrong.

What makes it more likely

Each entry states the mechanism, not just the association — an association without a mechanism is not something you can act on.

  • Physical injury. Collision with decor, a jump, rough netting or a fight causes local trauma and swelling. This is the commonest cause of a single affected eye.
  • Poor water quality. Chronic exposure to ammonia, nitrite or high nitrate is associated with bilateral cases, presumably through systemic effects and irritation.
  • Systemic bacterial infection. Infection can produce fluid accumulation behind both eyes, and often accompanies other signs such as reddening or abdominal swelling.
  • Gas supersaturation in the water. Dissolved gas coming out of solution in the tissues can present as bulging eyes with visible bubbles — an uncommon but genuine cause, usually following a plumbing fault or a pressurised water supply.

Treatment categories

AquariumHQ names classes of response and never doses. Product concentrations differ between markets and formulations, several actives are temperature- and pH-dependent, and the invertebrate and plant tolerances that decide whether the rest of the tank survives are not printed on the label. Correct the husbandry

Correcting water quality and removing physical hazards resolves the majority of cases, and is necessary regardless of cause. Injuries heal in clean water and do not in dirty water.

Water-change regime

Sustained improved water quality is the main supportive measure while the swelling resolves, which typically takes weeks.

Antibacterial

Adds a chemical to the water

Indicated where both eyes are affected alongside systemic signs, or where a local infection is established and not improving. It is not a first response to an injured eye in a clean tank.

What it can damage: Antibacterial products suppress nitrifying bacteria and can cause an ammonia spike, which is counterproductive where poor water quality was the cause. Several affect invertebrates.

  • Use before water testing

Veterinary referral

Appropriate where the eye is severely affected, where the cornea has become cloudy or ruptured, or where the condition is not improving. Loss of the eye is possible and a veterinarian can advise on managing it.

Isolation and quarantine

  • Isolation is worth considering where the cause appears to be aggression, because it removes the cause.
  • A fish with impaired vision may struggle to compete for food, and separate feeding or housing can help it maintain condition.

When a vet is the right call

  • Both eyes affected with systemic signs
  • Cloudy, ruptured or severely damaged eye
  • No improvement over several weeks of good water quality
  • Several fish affected, which points at the system

Prevention

  • Remove sharp decor and provide open swimming space to reduce collision injuries.

  • Address aggression and provide cover for harassed fish.
  • Cover the tank for species that jump.
