Correct the husbandry
Correcting water quality and reducing crowding raises the proportion of fish that clear the infection themselves, and reduces the secondary infections that follow the skin damage.
Ichthyophthirius multifiliis has a life cycle with three parts. The feeding stage sits under the epithelium of the fish and is the white spot you can see; it is physically protected there and no treatment reaches it. It then drops off, encysts on a surface in the tank, and divides into hundreds of free-swimming infective cells that must find a fish within a day or two or die. Only that last stage can be killed. The whole cycle is temperature-dependent, running in a few days in warm water and over weeks in cold, which is the single most important thing to understand about treating it: the calendar is set by the water, not by the fish.
Short answer
Whitespot is a ciliate parasite that embeds itself under the fish's outer skin layer, where it appears as discrete white specks roughly the size of a grain of salt. The parasite is only vulnerable to treatment during the free-swimming stage of its life cycle, which is why treatment has to continue for days after the spots disappear — and why the spots vanishing is not evidence that it has gone.
Recognisable by eye
The appearance is specific enough that a careful keeper can reach a working identification from the tank, though confirmation still improves the odds.
Water quality is the cause, the contributing factor or the deciding variable in the great majority of aquarium health problems. These steps come before any treatment decision, and for several conditions they are the entire treatment.
Every entry names the observation that separates it from this record. Work through these before deciding what you are treating — the most expensive mistake in fish health is confident treatment of the wrong thing.
How to tell it apart: Small white bumps confined to the gill covers and the leading edge of the pectoral fins of a male cyprinid — a danio, barb, minnow or goldfish — in breeding condition. They are symmetrical, they appear on males only, and they are normal. Whitespot has no such symmetry and does not respect the sexes.
How to tell it apart: Look at the fish from a second angle. Anything that moves relative to the fish when your viewpoint changes was never on the fish.
How to tell it apart: Epistylis colonies are tufted and slightly fuzzy at the edges rather than sharply defined, are often larger and more irregular than whitespot, and frequently sit on top of an existing sore or reddened area. The decisive difference is behavioural: Epistylis does not respond to antiparasitic treatment, so an outbreak that looks like whitespot and does not improve on treatment should be reconsidered rather than treated harder.
How to tell it apart: Velvet is a fine dusting rather than discrete spots, gold or rust rather than white, and is best seen by shining a torch along the flank in a darkened room. Whitespot specks can be counted; velvet cannot.
How to tell it apart: Lymphocystis growths are larger, irregular and cauliflower-textured, appear on fin rays and body margins, and grow slowly over weeks. Whitespot spreads over days and the individual spots stay a uniform small size.
How to tell it apart: Flagellate infections produce a blue-grey film and heavy mucus rather than countable spots, together with the same flashing behaviour. Distinguishing them needs a microscope, and the flashing on its own does not.

Raising temperature within the species' range compresses the parasite's life cycle so that the treatable free-swimming stage arrives sooner and the infection resolves in less time.
What it can damage: Warmer water holds less dissolved oxygen and increases the fish's oxygen demand at the same time. Coldwater and subtropical species are harmed by the temperature itself.
Proprietary whitespot treatments target the free-swimming infective stage. They cannot reach the feeding stage under the fish's skin, which is why the treatment period is governed by the parasite's life cycle rather than by the appearance of the fish.
What it can damage: Most whitespot treatments are toxic to shrimp, crayfish and snails, several stain silicone and decor, malachite-green formulations damage some plants, and copper-based products remain in substrate and silicone long after the water tests clear. Treatments that reduce dissolved oxygen make gill involvement worse.
Quarantining new arrivals for several weeks before they join an established tank is the only reliable way to keep whitespot out of a system, and is more effective than any treatment applied afterwards.
